This page explains what this website collects and what we do with it.

What you send us

When you fill in a form, we receive the details you type: your name, phone number, email, preferred office, time and language, and your message. We use them to call or write back to you about a visit.

Please don't put medical details in a form. For anything urgent, call us.

Your records

We keep your records private and share them only as the law allows, for example with your plan to bill a visit or with another doctor when we refer you. Your chart is the same at all six offices, and only the staff who care for you can see it.
